Your Sharp Health Plan member ID number, which identifies you as the main contact on the account.

Automatic bank withdrawals

Download, complete and send us the pre-authorization form below, and we can automatically withdraw your monthly premium payment for you on the 3rd of every month. (If the 3rd falls on a weekend or a holiday, the withdrawal will occur on the next business day.)

Creating an online account makes it easy to pay your bill.

  • Pay your premium bill by credit card, debit card or e-check.
  • Set up convenient automatic payments. Remember to update your automatic payments if your premium amount changes or your health coverage is terminated for any reason.
  • View your online transaction history.
  • Please note: if more than one person in your household is enrolled in Sharp Direct Advantage, each person will need to set up their own payment portal account.
