Yearly “Wellness” visits

Each year, you are eligible for a “Wellness” visit at no cost. That means you’ll have a $0 copay when you schedule your visit with a provider in your plan network. Yearly “Wellness” visits include:

  • A personalized prevention plan
    This is designed to help prevent disease and disability based on your current health and risk factors. Your provider will ask you to fill out a questionnaire, called a “Health Risk Questionnaire,” as part of this visit. Answering these questions can help you and your provider develop a personalized prevention plan to help you stay healthy and get the most out of your visit. It can also include:

    • A review of your medical and family history
    • Developing or updating a list of current providers and prescriptions
    • Height, weight, blood pressure, and other routine measurements
    • Personalized health advice
    • A list of risk factors and treatment options for you
    • A screening schedule (like a checklist) for appropriate preventive services
    • Details about coverage for screenings, shots, and other preventive services
    • Advance care planning
  • A cognitive impairment assessment
    The cognitive impairment assessment is performed to look for signs of memory problems including Alzheimer's disease or dementia. The assessment will also check for depression and other mood disorders. Your provider may order other tests, if necessary, depending on your general health and medical history.
  • TIP: When scheduling an appointment for a preventive care service, be specific that it is a “preventive care” visit so they know you shouldn’t be charged at the time of service. You can also download our free preventive care checklists and take them to your appointment. If medical symptoms, concerns or conditions are discussed at a preventive care visit, it could be considered a medical diagnosis or treatment and may require a copayment or deductible. You will need to schedule a separate office visit to address these issues.

Free fitness resources

Get a no-cost gym membership through the Silver&Fit® program and an at-home fitness kit that includes a Fitbit® Inspire. The gym membership will give you access to facilities such as 24 Hour Fitness, Curves, Jazzercise, LA Fitness and more throughout San Diego. If you prefer to exercise at home, there are 35 available at-home exercise programs including the new Fitbit® Connected! Kit that can be mailed directly to your home.

After-Hours Nurse Advice

After-hours and on weekends, registered nurses are available to give health advice to you over the phone through Sharp Nurse Connection®. Get answers to your questions about an illness or injury, learn more about when and where to seek care, and receive advice about other health concerns. Call 1-855-562-8853 from 5 pm – 8 am on weeknights and 24 hours on weekends.

Emergency Travel Services

When you’re traveling 100+ miles away from home or abroad, you can still get the care you need. Our out-of-area emergency services do not require an authorization from your primary care physician. Available services include medical consultations, evaluation and referral, prescription assistance and more.

Vision and hearing coverage

Vision Service Plan (VSP) Choice is included in Sharp Direct Advantage plans, so you can enjoy access to benefits such as annual routine eye exams and an allowance for glasses or contacts every 24 months. VSP has more than 400 providers throughout San Diego County to ensure that you can find the care you need close to your home.

Your plan may also include a hearing allowance every 2–3 years through San Diego Hearing Centers and HearUSA. Log in to your Sharp Health Plan online account to view your plan details for more information.

Acupuncture and chiropractic

Your plan may include acupuncture and chiropractic services through American Specialty Health. Your primary care physician can also refer you for treatments through your medical group.
