Steps to enroll online

Before you start, we recommend that you review the pre-enrollment checklist first.

Step 1: Check your eligibility.

To join a Sharp Direct Advantage plan, you must:

  • Be in an eligible enrollment period
  • Reside in San Diego County
  • Be entitled to Medicare Part A and enrolled in Medicare Part B, or be enrolled in Medicare Part A and Part B

Step 2: Choose a plan.

Step 3: Gather your information.

You'll need:

  • Your Medicare card (red, white and blue card)
  • Your primary care doctor's name and their medical group

If you do not currently have a primary care physician, we can help you find one.

Step 4: Start your enrollment form.

Use Google Chrome or Firefox as your web browser. Safari users may encounter issues with the form.

Prefer to enroll offline? You can also enroll by mail, by phone or one-on-one instead.

Sharp Direct Advantage is offered by Sharp Health Plan. Sharp Health Plan is an HMO with a Medicare contract. Enrollment with Sharp Health Plan depends on contract renewal. Read the full disclaimer.
