How to check your coverage

Two easy ways to access your coverage documents.

Your coverage documents tell you what’s covered, and what’s not covered, as part of your plan benefits. Coverage documents include your Evidence of Coverage, Summary of Benefits and Coverage and any other coverages or riders you added when enrolling with us.

Here are two ways to find your Sharp Health Plan coverage documents.

• Your Sharp Health Plan online account
  

  Review your coverage documents from your online account. When logged in, you can also access more self-service features.

• Forms, guides and resources
  

  Find coverage documents in our Forms and guides homepage. We also post important forms, our Medicare member resource guide, and much more.
