Enrollment steps

Follow these steps to apply online.

Step 1

Check your eligibility.

To join a Sharp Direct Advantage plan, you must:

  • Be in an eligible enrollment period
  • Reside in San Diego County
  • Be entitled to Medicare Part A and enrolled in Medicare Part B, or be enrolled in Medicare Part A and Part B
Step 2

Choose a Sharp Direct Advantage plan.

Before you begin your online enrollment form, you will need to know which plan you’d like to enroll in.

We offer plans for Medicare-eligible residents of San Diego, former Sharp HealthCare employees, and an exclusive plan for City of San Diego retirees.

Step 3

Gather your information.

Two items you need before you begin the enrollment form:

1. Your red, white and blue Medicare card

2. Your primary care physician’s name and plan medical group

If you do not currently have a primary care physician or would like to choose a different one, we can help you find one.

Step 4

Start your enrollment form.

When you are ready to enroll in a Sharp Direct Advantage plan, you will need to create a user account to start online enrollment.

Once you have an account, you will be able to begin your form, and return later to complete it.
